The 12 sub scoresheet is used in all competitions in Australia. Below are images of the scoresheet we use in our competition. Note that on the front of the scoresheet all the match details are contained as well as a VOTES panel. The votes are allocated by the 1st referee of the duty team for the selection of the MVP (Most Valuable Player) at the end of the season.
